Exit the DuoSwim app by swiping the application away and tapping on the app icon to reboot the app. If this doesn't solve your problem, try deleting the app (don't worry all your data is saved to your account) and re-install.
If your Apple Watch is paired you your iphone you simply just tab on Start then Send to Watch. Make sure the Apple Watch app is open before trying to send to workout.
You can manage your subscription through your Apple ID. Go to settings on your iphone, tab on your profile then select Subscriptions.
